D. J. Fazackerley was commissioned a 2nd Lieutenant in the Royal Field Artillery Special Reserve on 23 June 1917. He attained the rank of Acting Lieutenant-Colonel on 24 July 1945. He later served in the Lancashire (West) Army Cadet Force. His group of medals spans a remarkable 40 years of active and voluntary service; from a Lieutenant in 1917 to a Lieutenant-Colonel in the Army Cadet Force in 1949.
