D.S.M. London Gazette 27 June 1917. ‘... in recognition of their services in vessels of the Auxiliary Patrol between 1st February and 31st December 1916’.

Skipper Charles Slapp, R.N.R., of the H.M. Trawler
Ina Williams, was killed in action on 30 May 1917 when his vessel was mined off Berehaven. His name is commemorated on the Plymouth Naval Memorial.
